Advanced Practice Provider

Responsibilities

• Provide video-based, comprehensive clinical care including Acute Care, pediatrics, and mental health
• Work regular weekend day shifts (7a-7p Sat & Sun + 1 Weekday)
• Support physicians with Epic inbox management including labs, imaging, and patient questions
• Educate patients on appropriate treatments and care plans
• Prescribe medication and refills as appropriate
• Collaborate with Care Team Members to drive high-quality patient outcomes

Requirements

• 5+ years of clinical practice experience as a Family Nurse Practitioner in a Primary Care setting
• Licensed in FL, NJ, NY, MA and/or hold active NLC
• Minimum 1 year of experience using Epic EMR
• Currently enrolled or eligible for enrollment as a Medicare provider
• Experience in primary care, behavioral health, acute care, and chronic care management
• Proficiency with telemedicine technology
• Ability to work 36 hours per week (7a-7p Sat & Sun + 1 Weekday)
