Alert LPN

Responsibilities

• Monitor incoming vital signs, symptoms, and health indicators for a panel of patients enrolled in Cadence remote monitoring
• Triage alerts and abnormal readings against established clinical criteria
• Contact patients with out-of-range readings to gather additional clinical information
• Coordinate with nurses and nurse practitioners to route flagged patients into interventions
• Inspect AI-surfaced screening output and AI-drafted documentation for clinical accuracy
• Document every patient interaction in the clinical record and build care summaries

Requirements

• Active LPN/LVN license with compact licensure
• Minimum 5 years of experience practicing as an LPN/LVN
• Minimum 2 years of experience in a cardiology setting
• Experience in remote patient monitoring or telemedicine
• Experience with EMR-based clinical documentation
• Fluency with AI-assisted clinical tools
