Care Coordinator

Responsibilities

• Communicate with patients via text and phone to facilitate program engagement and follow-up
• Update patient records in Electronic Health Records (EHRs) and communicate care plans to clinic partners
• Maintain a collaborative care registry to track patient follow-up and clinical outcomes
• Facilitate outside referrals for community-based social services
• Collaborate with OB clinic staff to ensure coordinated care and support
• Report directly to the Lead Care Coordinator on patient engagement and outcomes

Requirements

• Bachelor's or Master's degree in nursing, social work, or psychology
• Strong understanding of women's mental health conditions and treatments
• Excellent communication, organizational, and problem-solving skills
• Ability to engage and educate patients in a compassionate manner
• Proficiency in using EHR systems and Google Suite
