About the role
Responsibilities
- Conduct scheduled telehealth nursing visits for members managing chronic cardiometabolic conditions
- Perform clinical intake, assessments, and medication reconciliation
- Provide member messaging support, education, and coaching
- Identify and communicate acute clinical findings or medication-related concerns for escalation
- Maintain accurate and timely clinical documentation in electronic health records
- Ensure services are provided within professional scope of practice and clinical protocols
Requirements
- Active, unrestricted Registered Nurse license in good standing
- Bachelor of Science in Nursing (BSN) required
- Minimum of five years of clinical nursing experience
- At least one year of experience in telehealth, virtual care, or ambulatory care
- Nurse Licensure Compact (NLC) multistate license or privilege required
- Proficiency with electronic health records and virtual care technology
Preferred Qualifications
- Certified Diabetes Care and Education Specialist (CDCES) certification
- Two or more years of experience supporting individuals with diabetes, obesity, hypertension, or dyslipidemia
