About the role
Responsibilities
- Address patients' clinical and pharmacy-related needs through industry-leading interventions
- Collaborate with a multidisciplinary team including providers, nurses, social workers, and care coordinators
- Support members in Case and Care Management and Virtual Primary Care Programs
- Provide pharmacy benefits navigation, treatment guidance, and medication support
- Manage chronic diseases through direct interaction and community resource guidance
- Partner with clinical leadership to develop innovative pharmacy interventions and assess solutions to improve population health outcomes
Requirements
- Clinical Pharmacist expertise
- Ability to work in a remote capacity with members across the United States
- Experience in multidisciplinary care collaboration
