Job description

Responsibilities

  • Provide integrated support to patients and physicians to improve medication adherence and promote evidence-based care
  • Review clinician patient care notes and assessments for prior authorization approvals and care plan recommendations
  • Develop and refine disease and drug specific clinical protocols
  • Monitor patient adherence using PDC reports, assessment data, and mobile platform data
  • Evaluate EMR for drug interactions, medication selection, labs, vitals, and allergies
  • Recommend alternative therapeutic drug regimens and disease specific treatments
  • Perform medication therapy management (MTM) including monitoring regimens and advising clinicians
  • Conduct clinical interventions such as comprehensive pharmacist consultation and motivational interviewing
  • Oversee technician daily activities and verify accuracy of delivery coordination and prior authorizations
  • Identify and resolve insurance-related issues and financial assistance opportunities
  • Maintain current registration and comply with local, state, and federal drug laws

Requirements

  • Doctor of Pharmacy (PharmD) or equivalent advanced degree
  • Licensed Pharmacist in the state of Oregon
  • Proficiency in Microsoft Office (Word, Excel, PowerPoint)
  • Strong technical and clinical skills
  • Ability to identify barriers to medication adherence

Preferred Qualifications

  • 1+ years of experience in clinical pharmacy
  • Knowledge of specialty pharmacy disease states (Oncology, Transplant, Hepatitis C, HIV, MS, etc.)
  • Experience with EPIC, QS1, or WILLOW software
