About the Company
Form Health is a virtual obesity medicine clinic delivering multi-disciplinary evidence-based obesity treatment through telemedicine. Founded in 2019, Form Health is a venture-backed innovative startup with an experienced clinical and leadership team dedicated to empowering patients and leading in Obesity Medicine.
Responsibilities
- Provide direct patient care focusing on evidence-based obesity treatment and chronic weight management
- Manage a personal patient panel and collaborate with Registered Dietitians
- Provide treatment planning and medical management support for complex cases
- Prescribe non-controlled obesity medications
- Ensure clinical documentation and treatment pathways are followed consistently
- Assist in the professional development of APPs through feedback and clinical support
- Participate in team-based care initiatives and quality improvement efforts
- Participate in a rotating after-hours call schedule
Requirements
- MD or DO degree with board certification
- ABOM diplomate or board certified in Endocrinology
- Licensed to practice and prescribe in AL, FL, GA, IN, KY, MI, MS, NC, OH, SC, or TN
- Must hold an LOQ (Letter of Qualification) with the IMLC
- Experience with insurance payer credentialing
- Prior experience conducting telemedicine visits
- Experience working collaboratively with APPs and Registered Dietitians
Preferred Qualifications
- Multiple state licenses
- Bilingual with medical fluency in Spanish
