Healthcare Advocate

Responsibilities

• Develop trusting relationships with patients through listening and respect
• Assess medical, social, and environmental factors to pinpoint barriers and needs
• Create individualized care plans integrating medical, social, and behavioral supports
• Connect patients to local services such as transportation, meal delivery, and home care
• Analyze medical bills and assist with claim appeals or explanations
• Identify medication cost savings and co-pay assistance options
• Organize and maintain medical records and provider communication
• Document interactions and patient progress in Baba's systems
• Collaborate with clinical, product, and operations teams to optimize workflows
• Provide feedback on platform features and tools to improve care capabilities

Requirements

• Associate's or Bachelor's degree in Nursing, Social Work, Public Health, Health Administration, or related field
• Active RN, LSW/LMSW/LCSW, CCM/ACM, CHW, LPN/LVN, or other relevant clinical or care management credential preferred
• 2+ years of experience in care navigation, patient advocacy, care management, case management, home health, discharge planning, utilization management, or clinical social work
• Solid knowledge of Medicare rules, eligibility, benefit structures, claims, and appeals
• Deep understanding of Social Determinants of Health (SDOH)
• Empathy, patience, and strong communication skills
• Comfort working with digital tools, EMRs, and care-management software
• Organized, proactive, and and able to juggle multiple priorities
