Implementation Manager

Responsibilities

• Run the ECC onboarding sequence end-to-end for each new practice
• Hold the working timeline across multiple concurrent implementations
• Coordinate parallel implementation and clinical workstreams
• Build agendas and prepare meeting materials and decks
• Facilitate meetings with the Program Director or independently
• Serve as the primary day-to-day point of contact for partner practice leadership
• Maintain a single source of truth for deadlines and open items
• Coordinate with internal Finance, Legal, Data, and People Operations
• Identify process improvements and propose durable fixes

Requirements

• 5+ years of experience in implementation, program or project management, client success, or provider operations
• Demonstrated experience running multi-stakeholder implementations to a defined timeline
• Excellent written and verbal communication skills
• Strong organizational discipline and follow-through
• Advanced skills with MS Office Suite (Outlook, Teams, SharePoint, Excel, PowerPoint)
• Willingness to travel occasionally for practice kickoffs and on-site sessions
• Ability to work effectively in a remote environment with dedicated space and high-speed internet

Preferred Qualifications

• Experience with nephrology, chronic care management, or other CMS care management programs
