Responsibilities

• Provide a welcoming and informative experience for all new members during their enrollment process
• Ensure all pertinent medical information, including medication lists and medical history, is collected accurately
• Assist the Provider Team in the assessment and enrollment of new members
• Educate new members on program details, benefits, and device usage expectations
• Order remote monitoring devices for members
• Perform high-quality, high-volume calls while maintaining a culture of kindness and compassion

Requirements

• Current, active, unrestricted LPN multi-state compact license in good standing
• Minimum of 2 years hands-on nursing experience
• Reliable high-speed internet
• Strong time management and self-motivation skills
• Ability to work independently in a fast-paced environment
• Professional and effective communication style

Preferred Qualifications

• Telehealth and chronic care management experience
• EHR experience, specifically AthenaHealth
• Fluency in English and Spanish
• Experience navigating multiple computer applications and using Macbooks
