About the role
Responsibilities
- Monitor incoming vital signs, symptoms, and health indicators for a panel of patients enrolled in Cadence remote monitoring
- Triage alerts and abnormal readings against established clinical criteria
- Contact patients with out-of-range readings to gather additional clinical information
- Coordinate with Cadence nurses and nurse practitioners to route flagged patients
- Inspect AI-surfaced screening output and AI-drafted documentation for clinical accuracy
- Document every patient interaction in the clinical record and build care summaries
Requirements
- Active LPN/LVN license with compact licensure
- Minimum 5 years of experience practicing as an LPN/LVN
- Minimum 2 years of experience in a cardiology setting
- Experience in remote patient monitoring or telemedicine
- Experience with EMR-based clinical documentation
- Fluency with AI-assisted clinical tools
