About the role
Responsibilities
- Guide members through routine and high-risk pregnancy experiences via telephonic support
- Develop personalized care plans for fertility planning, prenatal care, delivery preparation, and postpartum recovery
- Collaborate with a multidisciplinary team of providers, care coordinators, and support staff
- Serve as a trusted advocate for members, answering questions and addressing concerns
- Help members navigate health benefits and connect them to appropriate resources
Requirements
- Strong clinical expertise in maternity care
- Ability to provide evidence-based, holistic care
- Experience in patient-centered case management
- Ability to deliver integrated remote care
