Mental Health Therapist (LCSW/LPC) (Full-Time & Remote) - Tennessee License

Responsibilities

• Act as mental health support for referred patients via the Diana Health platform
• Conduct telehealth mental health visits via warm hand-off process
• Provide ongoing mental health treatment and refer out as needed
• Complete timely clinical documentation within EHR system
• Collaborate with CNM/OB/Gyn on plan of care and medication escalation
• Collaborate with Health Coaches and Care Navigators to deliver integrated care
• Provide staff training on behavioral health workflows
• Lead expert talks and support groups

Requirements

• Licensed Clinical Social Worker (LCSW) or Marriage Family Therapist (MFT) with current Tennessee licensure
• 2+ years of experience providing mental health support to a similar client panel
• Excellent communication, interpersonal, and organizational skills
• Strong computer skills and familiarity with EMRs
• Preference for specialized modalities such as postpartum, trauma, EMDR, or IFS (non-CBT/DBT)
