About the role
Responsibilities
- Act as mental health support for referred patients via the Diana Health platform
- Conduct telehealth mental health visits via warm hand-off process
- Provide ongoing mental health treatment and refer out as needed
- Complete timely clinical documentation within EHR system
- Collaborate with CNM/OB/Gyn on plan of care and medication escalation
- Collaborate with Health Coaches and Care Navigators to deliver integrated care
- Provide staff training on behavioral health workflows
- Lead expert talks and support groups
Requirements
- Licensed Clinical Social Worker (LCSW) or Marriage Family Therapist (MFT) with current Tennessee licensure
- 2+ years of experience providing mental health support to a similar client panel
- Excellent communication, interpersonal, and organizational skills
- Strong computer skills and familiarity with EMRs
- Preference for specialized modalities such as postpartum, trauma, EMDR, or IFS (non-CBT/DBT)
