Responsibilities
- Guide newborn and pediatric members through complex medical and behavioral health situations
- Partner with a multidisciplinary clinical team to deliver integrated remote care
- Create cohesive care plans and navigate available benefits and resources
- Support members through complex care, disease, and acute case management
