About the role
About the Company
Baba is rebuilding healthcare for older adults. By pairing older adults with expert advocates, Baba helps families navigate complex medical systems, research specialists, and manage insurance and medications.
Responsibilities
- Conduct telehealth-based E/M or Annual Wellness Visits (AWVs) to establish medical necessity for care navigation services.
- Identify high-risk conditions and unmet social needs during intake visits.
- Document findings in the platform using accurate coding, including SDOH Z-codes and diagnoses.
- Develop and approve individualized care plans in collaboration with care advocates and nurse care managers.
Requirements
- Completion of an accredited Nurse Practitioner program
- Active Nurse Practitioner or APRN license in good standing (licensed in multiple states)
- 2+ years of experience in primary care, telehealth, or care management
- Bilingual proficiency in Mandarin
- Familiarity with Medicare programs and SDOH documentation
- Proficiency with EMRs and telehealth workflows
What we look for

- Active NP or APRN license in multiple states
