Nurse Practitioner

Responsibilities

• Manage a panel of patients receiving remote care, making clinical decisions for heart failure, hypertension, and type 2 diabetes
• Initiate and titrate guideline-directed medical therapy (GDMT)
• Respond to clinical escalations from remote patient monitoring, including abnormal vitals and lab results
• Provide synchronous and asynchronous care via secure messaging and virtual visits
• Collaborate with physicians, RNs, and care teams to optimize patient outcomes
• Use multiple technology platforms and provide feedback to improve clinical operations

Requirements

• 5+ years experience treating chronic diseases (CHF, Hypertension, T2D)
• 3+ years of cardiology experience with a focus on heart failure management
• Active ANCC or AANP certification
• Master’s Degree as a Nurse Practitioner
• Active Compact Multi-State RN license
• Active NP licensure in at least two of: TX, MI, NC, CA, PA, NJ, CT, AL
• Stable home office Wi-Fi setup

Preferred Qualifications

• Experience in a CHF bridge clinic or T2D clinic
• Experience in a startup environment
• Experience with remote patient monitoring technology
• Comfort using AI-assisted clinical tools
