About the role
Responsibilities
- Manage a panel of patients receiving remote care, making clinical decisions for heart failure, hypertension, and type 2 diabetes
- Initiate and titrate guideline-directed medical therapy (GDMT)
- Respond to clinical escalations from remote patient monitoring, including abnormal vitals and lab results
- Provide synchronous and asynchronous care via secure messaging and virtual visits
- Collaborate with physicians, RNs, and care teams to optimize patient outcomes
- Use multiple technology platforms and provide feedback to improve clinical operations
Requirements
- 5+ years experience treating chronic diseases (CHF, Hypertension, T2D)
- 3+ years of cardiology experience with a focus on heart failure management
- Active ANCC or AANP certification
- Master’s Degree as a Nurse Practitioner
- Active Compact Multi-State RN license
- Active NP licensure in at least two of: TX, MI, NC, CA, PA, NJ, CT, AL
- Stable home office Wi-Fi setup
Preferred Qualifications
- Experience in a CHF bridge clinic or T2D clinic
- Experience in a startup environment
- Experience with remote patient monitoring technology
- Comfort using AI-assisted clinical tools
