Nurse Practitioner

Responsibilities

• Virtually evaluate patients entering the program and assist in creating a care plan based on patient specific needs and predesignated protocols
• Respond to alerts escalated from the nursing team related to patients' deviations in vitals and physiologic data, symptoms, and weight changes
• Independently manage changes in clinical status within the APP's scope of practice
• Escalate out of scope clinical changes to lead MD
• Liaise with the nursing team to ensure proper medications, labs, and virtual appointments are ordered and communicated
• Partner with the nursing team to ensure medications are reconciled on program entry and exit
• Serve as the point of contact for each patient's primary care teams
• Provide education to patients about healthy behaviors and self-management

Requirements

• 2+ years as a Board-Certified Nurse Practitioner specializing in heart failure and/or internal medicine
• Active license in California
• Ability to work from home in a HIPAA compliant environment
• Proficiency with multiple EMRs and communication applications including Google products

Preferred Qualifications

• Multiple state licenses
• Fluency in English and Spanish
• Previous experience overseeing complex chronic care programs
• Previous telehealth experience
