Palliative Care Transitions Specialist

Responsibilities

• Review incoming referrals for palliative appropriateness and classify them
• Confirm patient eligibility and coverage windows before care starts
• Own the transition from palliative care to hospice, coordinating goals-of-care discussions
• Track patients across transition points between palliative, hospice and primary care
• Complete individualized care plans and manage care using the EMR
• Serve as point of contact between providers, patients, families and facilities
• Track and document Chronic Care Management minutes accurately
• Document patient communications accurately and in a timely manner

Requirements

• Direct clinical or hands-on patient care experience in hospice or palliative care
• Working knowledge of palliative and hospice eligibility criteria and goals-of-care concepts
• Comfortable on the phone for large portions of the day
• Highly organized, able to multitask across patients and workstreams
• Proficient with computers, Outlook and Microsoft Office
• Must pass a background check
