About the role
Responsibilities
- Proactively monitor patient vitals, symptoms, and clinical data to identify care gaps
- Develop, implement, and manage individualized care plans aligned with physician recommendations
- Engage with patients via phone and secure messaging to provide clinical support
- Conduct virtual follow-up visits to support program onboarding and medication adherence
- Document all clinical interactions accurately in EMR systems
- Partner with interdisciplinary care teams to manage chronic conditions like CHF, hypertension, and type 2 diabetes
- Contribute to the refinement of clinical workflows and processes
Requirements
- Active Multi-state RN Compact State Licensure
- Associate Degree in Nursing Science
- Strong clinical acumen and nursing process skills
- Proficiency with EMR platforms (Athena and EPIC preferred)
- Technical fluency with Notion, Zendesk, and G Suite
- Stable home wifi with strong upload/download speeds
- Exceptional communication and interpersonal skills
Preferred Qualifications
- 5+ years of recent clinical experience in an ICU/ER setting
- Prior experience in chronic care management programs
