Registered Nurse

Responsibilities

• Proactively monitor patient vitals, symptoms, and clinical data to identify care gaps
• Develop, implement, and manage individualized care plans aligned with physician recommendations
• Engage with patients via phone and secure messaging to provide clinical support
• Conduct virtual follow-up visits to support program onboarding and medication adherence
• Document all clinical interactions accurately in EMR systems
• Partner with interdisciplinary care teams to manage chronic conditions like CHF, hypertension, and type 2 diabetes
• Contribute to the refinement of clinical workflows and processes

Requirements

• Active Multi-state RN Compact State Licensure
• Associate Degree in Nursing Science
• Strong clinical acumen and nursing process skills
• Proficiency with EMR platforms (Athena and EPIC preferred)
• Technical fluency with Notion, Zendesk, and G Suite
• Stable home wifi with strong upload/download speeds
• Exceptional communication and interpersonal skills

Preferred Qualifications

• 5+ years of recent clinical experience in an ICU/ER setting
• Prior experience in chronic care management programs
