About the role
Responsibilities
- Conduct initiating telehealth-based E/M or Annual Wellness Visits (AWVs) to establish medical necessity for care navigation services
- Identify high-risk conditions and unmet social needs during intake visits
- Document findings in the platform using accurate coding, SDOH Z-codes, diagnoses, and risk factors
- Develop and approve individualized care plans to ensure alignment with medical treatment plans
Requirements
- M.D. or D.O. with an active, unrestricted U.S. medical license
- Broad multistate licensure (IMLC participation or 50-state coverage preferred)
- 2+ years of experience in primary care, telehealth, or care management
- Proficiency with EMRs, care management platforms, and telehealth workflows
- Bilingual Mandarin fluency required
- Familiarity with Medicare programs and SDOH documentation
Preferred Qualifications
- Board certification in Internal Medicine, Family Medicine, or Geriatrics
