Baba is rebuilding healthcare for older adults. By pairing older adults with expert advocates, Baba helps navigate the complexities of healthcare, including insurance appeals, specialist research, and medication coordination. The company is backed by top-tier investors and supports families across assisted living, nursing homes, and at-home settings.
Responsibilities
- Conduct initiating telehealth visits (E/M or Annual Wellness Visits) to establish medical necessity for care navigation services.
- Identify high-risk conditions and unmet social needs during intake.
- Document findings in the platform using accurate coding, including SDOH Z-codes and diagnoses.
- Develop and approve individualized care plans to ensure alignment with medical treatment plans.
Requirements
- M.D. or D.O. with an active, unrestricted U.S. medical license.
- Broad multistate licensure (IMLC participation or 50-state coverage preferred).
- Bilingual Spanish fluency required.
- 2+ years of experience in primary care, telehealth, or care management.
- Familiarity with Medicare programs and SDOH documentation.
- Proficiency with EMRs and telehealth workflows.
Preferred Qualifications
- Board certification in Internal Medicine, Family Medicine, or Geriatrics.
