Job description

Responsibilities

  • Monitor high-risk pediatric patients and ensure appropriate follow-up
  • Conduct biopsychosocial assessments to address behavioral, social, and emotional needs
  • Create, evaluate, and modify patient/family care plans
  • Provide interventions for mental health crises, behavioral issues, and family conflict
  • Facilitate case management for patients with mental health and co-occurring medical conditions
  • Resource external services such as social services agencies and behavioral specialists
  • Provide crisis intervention to de-escalate and stabilize patients and families
  • Offer psychoeducation on mental health diagnoses and treatment adherence
  • Collaborate with external care teams including schools, specialists, and child welfare agencies
  • Participate in interdisciplinary team consultations

Requirements

  • Master's degree in social work or a related field
  • LMSW licensure
  • 3-5 years of post-graduate experience in healthcare social work or behavioral health case management
  • Experience working with pediatric populations and family systems
  • Proficiency in motivational interviewing and assessment techniques
  • Knowledge of crisis prevention and intervention
  • Experience providing telehealth services
  • Proficiency in MS Office Suite and online platforms
  • Knowledge of trauma-informed care practices
  • Ability to work afternoons and evenings
  • Willingness to travel up to 10% for training

Preferred Qualifications

  • Bilingual in Spanish
