Job description
Responsibilities
- Monitor high-risk pediatric patients and ensure appropriate follow-up
- Conduct biopsychosocial assessments to address behavioral, social, and emotional needs
- Create, evaluate, and modify patient/family care plans
- Provide interventions for mental health crises, behavioral issues, and family conflict
- Facilitate case management for patients with mental health and co-occurring medical conditions
- Resource external services such as social services agencies and behavioral specialists
- Provide crisis intervention to de-escalate and stabilize patients and families
- Offer psychoeducation on mental health diagnoses and treatment adherence
- Collaborate with external care teams including schools, specialists, and child welfare agencies
- Participate in interdisciplinary team consultations
Requirements
- Master's degree in social work or a related field
- LMSW licensure
- 3-5 years of post-graduate experience in healthcare social work or behavioral health case management
- Experience working with pediatric populations and family systems
- Proficiency in motivational interviewing and assessment techniques
- Knowledge of crisis prevention and intervention
- Experience providing telehealth services
- Proficiency in MS Office Suite and online platforms
- Knowledge of trauma-informed care practices
- Ability to work afternoons and evenings
- Willingness to travel up to 10% for training

- Bilingual in Spanish
