Bilingual Care Coordinator, LPN

Responsibilities

• Perform monthly clinical check-ins and CCM touchpoints for assigned patient panels
• Conduct structured symptom assessments and evaluate changes in condition using clinical judgment
• Identify clinical red flags and escalate urgent issues via defined triage pathways
• Collaborate with provider teams on symptom changes, medication issues, and care coordination
• Reinforce provider-developed CKD and chronic disease care plans
• Document all patient interactions accurately in EHR and CCM platforms
• Track and log qualifying CCM time for billing compliance

Requirements

• Active LPN or Licensed Vocational Nurse (LVN) license
• Bilingual in Spanish and English
• 1–2 years of clinical experience in ambulatory care, population health, care coordination, or case management
• Proficiency with MS Office Suite (Outlook, Teams)
• Ability to work in a remote environment with a dedicated, private workspace
• Reliable high-speed internet (min 25 Mbps download / 10 Mbps upload)

Preferred Qualifications

• Experience in chronic disease management
• Prior experience in virtual care, remote nursing, or telehealth programs
• Familiarity with CCM regulatory requirements
• Strong patient education and motivational interviewing skills
