Form Health is a virtual obesity medicine clinic delivering multi-disciplinary evidence-based obesity treatment through telemedicine. Founded in 2019, Form Health is a venture-backed innovative startup dedicated to empowering patients and leading the field of Obesity Medicine at a national scale.

Responsibilities

• Provide direct patient care focusing on evidence-based obesity treatment and chronic weight management
• Manage a personal patient panel and collaborate with Registered Dietitians
• Provide treatment planning and medical management support for complex cases
• Prescribe non-controlled obesity medications
• Ensure clinical documentation and treatment pathways are followed consistently
• Assist in the professional development of APPs through feedback and clinical support
• Participate in team-based care initiatives and quality improvement efforts
• Participate in a rotating after-hours call schedule

Requirements

• MD or DO degree with board certification
• ABOM diplomate or board certified in Endocrinology
• Active medical license to practice and prescribe in NY, VA, or MA
• Experience with insurance payer credentialing
• Prior experience conducting telemedicine visits
• Experience working collaboratively with APPs and Registered Dietitians
• Excellent interpersonal skills for patient and team interactions

Preferred Qualifications

• Multiple state licenses
• Bilingual with medical fluency in Spanish
