Baba helps seniors and caregivers manage appointments, billing questions, and complex healthcare decisions, supporting thousands of families across assisted living, nursing homes, and at-home settings.

Responsibilities

• Develop trusting relationships with patients through listening, respect, and clarity
• Assess medical, social, and environmental factors to pinpoint barriers and needs
• Create individualized care plans integrating medical, social, and behavioral supports
• Connect patients to local services such as transportation, meal delivery, and home care
• Analyze medical bills and assist with claim appeals or explanations
• Identify medication cost savings, co-pay assistance, or formulary options
• Link patients to social support networks and community programs
• Organize and maintain medical records, care histories, and provider communication
• Coach patients on best practices for managing conditions and navigating care
• Document interactions and patient progress in Baba's systems
• Collaborate with clinical, product, and operations teams to optimize workflows

Requirements

• Bachelor's degree in Nursing, Social Work, Public Health, Health Administration, or related field
• 2+ years of experience in care management, patient advocacy, case management, or clinical social work
• Experience supporting older adults or Medicare populations
• Solid knowledge of Medicare rules, eligibility, benefit structures, claims, and appeals
• Deep understanding of Social Determinants of Health (SDOH)
• Proficiency with digital tools, EMRs, and care-management software
• Minimum 20 hours of availability per week, including 75% during standard business hours

Preferred Qualifications

• Active RN, LPN/LVN, LSW/LMSW/LCSW, CCM/ACM, or CHW credential
