Responsibilities

• Provide virtual care, primarily via CBT, to eligible patients diagnosed with cancer
• Conduct initial needs assessments to guide care throughout the cancer journey
• Care for patients in individual visits to address mental health needs and concerns
• Screen for unmet care needs, including clinical, medication, behavioral, and psychosocial issues
• Establish trusting relationships with patients and caregivers using active listening and empathy
• Consult with licensed clinical team members and participate in weekly interdisciplinary patient rounds
• Document all patient and caregiver interactions in the EHR system
• Integrate collaborative care programming into the larger Daymark care model

Requirements

• Master of Social Work (MSW) or MSW-equivalent degree
• Active Licensed Clinical Social Work (LCSW) license
• At least two years of clinical work experience
• Proficiency in CBT or similar therapeutic techniques
• Working knowledge of medical terminology
• Licensed in (or willing to be licensed in) Rhode Island, Massachusetts, and Connecticut
• Proficiency with Slack, Zoom, Google Suite, and EHR systems

Preferred Qualifications

• Previous experience working with patients living with cancer
