Nurse Practitioner

Responsibilities

• Conduct thorough chart reviews for PACE applicants to evaluate medical records and clinical documentation
• Present clinical cases to Site Medical Directors to support eligibility decisions
• Collaborate with Enrollment RNs to identify and resolve documentation gaps
• Provide clinical expertise for Health & Safety determinations and participate in committee meetings
• Conduct virtual clinical assessments to confirm diagnoses when required
• Review ILOC clinical summaries for regulatory compliance
• Support quality improvement initiatives to maintain high approval rates

Requirements

• Current, unrestricted Nurse Practitioner (NP) or Physician Assistant (PA) license in California
• Current DEA certification and furnishing/prescriptive authority in California
• Minimum 3 years of post-licensure clinical experience in geriatrics, long-term care, or PACE-adjacent settings
• Proficiency with geriatric assessment tools (BIMS, PHQ-9, MoCA, MMSE, ADL/IADL)
• Strong ILOC documentation skills meeting California Title 22 and 42 CFR Part 460 standards
• Proficiency with Epic EHR and Microsoft Outlook
• Understanding of CMS and DHCS regulations related to PACE eligibility
• Experience presenting complex cases in a fast-paced clinical environment

Preferred Qualifications

• Previous experience in PACE or similar programs for older adults
• Proficiency with telehealth and virtual assessment methodologies
• Familiarity with DHCS ILOC submission process
• Knowledge of behavioral health assessment in geriatric populations
• Bilingual Spanish/English
• Adult-Gerontology Primary Care or Acute Care NP certification
