Form Health is a virtual obesity medicine clinic delivering multi-disciplinary evidence-based obesity treatment through telemedicine. Founded in 2019, Form Health is a venture-backed innovative startup dedicated to empowering patients and leading in the field of Obesity Medicine.

Responsibilities

• Act as the primary point of contact for new patients via phone and email to guide them through enrollment.
• Support patients in completing initial enrollment tasks within the Core platform to reduce drop-off.
• Register and onboard a high volume of new patients by phone, collecting insurance details and ID cards.
• Set up patient charts in the electronic medical record (EMR) system.
• Verify patient insurance information to confirm active coverage and eligibility.
• Perform proactive outreach to encourage patients to complete outstanding onboarding steps.
• Provide feedback to Product, Engineering, and Marketing teams to improve operational tools.

Requirements

• At least two years of inbound phone experience in healthcare, patient enrollment, or high-volume customer service.
• Proven ability to manage high task volumes independently while meeting enrollment targets.
• Excellent verbal and written communication skills with an empathetic approach.
• Proficiency in computer platforms including Slack, Google Workspace, and EMR systems (e.g., Elation).
• Ability to remain professional during challenging calls or technical hurdles.
