Quality Outreach Specialist

Responsibilities

• Conduct patient outreach calls to address medication non-adherence and identify barriers to care
• Connect patients with essential care services to close critical gaps in care
• Perform medical record reviews to identify and document evidence of clinical gap closures
• Submit gap in care closure details to payers and joint venture partners
• Provide subject matter expertise for the development of educational tools and learning sessions
• Identify learning opportunities for providers based on outreach and medical record review outcomes
• Support the creation and review of educational materials for external provider partners

Requirements

• Medical Assistant, LPN, CNA, EMT, or Pharmacy Technician license/certification required
• 3+ years of experience closing HEDIS measures or Star Rating gaps
• Experience with quality measure chart reviews
• Proficiency in HIPAA compliance
• Strong proficiency in Microsoft Office Suite (Excel, PowerPoint, Word, Outlook)
• Ability to manage deadlines and handle multiple tasks simultaneously
• Strong verbal and written communication skills

Preferred Qualifications

• Bachelor's or Undergraduate degree
• Inpatient or outpatient clinical experience
• Strong experience with Electronic Medical Records (EMR)
• Experience working with large files and data sets
