Job description

Responsibilities

  • Provide call-based care and clinical nutrition management to patients in a Remote Patient Monitoring (RPM) program
  • Assess member health regimens and interpret blood glucose and CGM readings
  • Craft individualized, compassionate meal plans based on member health needs and access
  • Support nutrition-related escalations and provide data-driven education
  • Collaborate with RNs and Accountability Coaches to reinforce clinical goals and SMART goal progression
  • Adhere to company policies and standard operating procedures to ensure clinical quality

Requirements

  • BS in Nutrition
  • Valid, unencumbered state RD license (Priority states: TN, OH, MS, LA)
  • CDR and CDCES certification required
  • Ability to work remotely with a clean, quiet workspace and solid internet connection
  • Minimum 20 hours of weekly availability

Preferred Qualifications

  • 3+ years managing patients with diabetes
  • Familiarity with continuous glucose monitors (CGMs)
  • Knowledge of behavior change theory
  • Training in Motivational Interviewing
  • Spanish-speaking proficiency
