Responsibilities

• Provide intensive, ongoing medical nutrition therapy support to patients via regular video visits
• Establish professional, non-judgmental, and empathetic relationships with patients
• Create customized nutrition plans based on individual needs
• Provide nutrition education and counseling regarding diabetes self-management, healthy eating, and behavior change
• Act as a clinical specialist in diabetes, including data interpretation using Continuous Glucose Monitors (CGMs) or glucometers
• Support patients with complex co-morbidities such as Chronic Kidney Disease (CKD) and Cardiovascular conditions
• Review and offer feedback on patients’ daily food photos and support via daily text messaging

Requirements

• Registered Dietitian with 3-5 years of experience in counseling and behavior modification
• Experience working with patients with diabetes (primarily Type 2 Diabetes)
• Experience in medical weight loss, including working with patients on weight loss medication and meal replacement plans
• Proficiency in cognitive-behavioral therapy counseling techniques

Preferred Qualifications

• Certified Diabetes Care and Education Specialist (CDCES)
