Registered Nurse

Responsibilities

• Proactively monitor patient vitals, symptoms, and clinical data to identify care gaps
• Develop and manage individualized care plans aligned with physician recommendations
• Engage with patients via phone and secure messaging to provide clinical support
• Conduct virtual follow-up visits to support program onboarding and medication adherence
• Document clinical interactions accurately in EMR systems
• Partner with interdisciplinary care teams to manage chronic conditions like CHF and diabetes
• Contribute to the refinement of clinical workflows and processes

Requirements

• Active Compact Multi-State RN license
• Active Illinois state RN license
• Associate Degree in Nursing Science
• Skilled in nursing processes and patient assessment
• Technical fluency with platforms like Notion, Athena, EPIC, Zendesk, and G Suite
• Stable home wifi with strong upload/download speeds

Preferred Qualifications

• 5+ years of recent clinical experience in an ICU/ER setting
• Prior experience in chronic care management programs
• Experience with Athena and EPIC EMR
