Telehealth Heart Failure Registered Nurse

Responsibilities

• Triage patients using protocols to determine necessary measures and escalation levels
• Provide virtual nursing assessments to evaluate symptoms, medication adherence, and disease progression
• Educate patients and caregivers on heart failure medications and potential side effects
• Monitor patient vital signs and survey responses to identify and document changes in condition
• Document all clinical interactions and interventions
• Manage incoming calls, including answering, directing, and following up
• Recognize urgent or emergent situations and initiate emergency interventions as directed

Requirements

• Bachelor's Degree in Nursing
• Current, active, unrestricted Registered Professional Nurse license in a compact state
• Minimum 3 years experience in cardiology, heart failure management, or critical care nursing
• Strong knowledge of GDMT (Guideline-Directed Medical Therapy)
• Experience with patient phone triage (in-office or telemedicine)
• Proficiency with multiple EMRs and communication applications like Google products
• Ability to work from home in a HIPAA compliant environment

Preferred Qualifications

• Advanced certification (CCRN, CHFN, RN-BC)
• Fluency in English and Spanish
