Advanced Practice Provider (APP)

Responsibilities

• Provide text-based and video-based comprehensive clinical care including acute care, pediatrics, and mental health
• Conduct video-based Acute Care, Primary Care, and Behavioral Health patient visits
• Support physicians with Epic inbox management including labs, imaging, and patient questions
• Educate patients on appropriate treatments and care plans
• Prescribe medication and refills as appropriate
• Collaborate with Care Team Members to drive high-quality patient outcomes

Requirements

• 5+ years of clinical practice experience as a Family Nurse Practitioner or Physician Assistant in a Primary Care setting
• Active license in New Jersey
• Minimum 1 year of experience using Epic EMR
• Currently enrolled or eligible for enrollment as a Medicare provider
• Experience in primary care and behavioral health, treating both acute and chronic conditions
• Proficiency with telemedicine technology
• Ability to commit to 40 hours per week (36 clinical hours)
