Job description

Responsibilities

  • Provide medication management including reconciliation, dose optimization, and adherence counseling
  • Collaborate with interdisciplinary care teams to develop individualized care plans
  • Monitor medication regimens for therapeutic goals, adverse effects, and drug interactions
  • Identify opportunities for drug therapy optimization and cost savings
  • Perform clinical assessments and educational counseling for high-priority patients
  • Support prior authorizations and appeals by providing clinical justification
  • Participate in quality improvement initiatives and clinical outcome studies

Requirements

  • Doctor of Pharmacy degree (PharmD) from an accredited school
  • Licensed pharmacist in Colorado (CO)
  • Postgraduate residency or one year of clinical service under a collaborative practice agreement
  • Proficiency in Microsoft Office tools (Outlook, Teams, Word, Excel, PowerPoint)
  • Strong clinical knowledge of pharmacology and FDA regulations
