Bilingual/Spanish Speaking Health Coach

Responsibilities

• Conduct outbound telephonic patient outreach and provide one-on-one coaching for chronic conditions like type 2 diabetes and hypertension
• Help patients execute personalized care plans using motivational interviewing and SMART goals
• Conduct comprehensive assessments of health status, lifestyle, and nutritional habits
• Educate patients on disease management, medication adherence, and symptom recognition
• Monitor patient progress and health outcomes through remote monitoring tools
• Document all patient interactions and identify social barriers to care in the Cadence platform
• Collaborate with clinicians and support teams using Google Workspace and Slack

Requirements

• Active NBHWC health coaching certification
• At least 1 year of experience as a health coach
• Experience providing patient education for chronic condition management
• Fluency in Spanish
• Proficiency with AI-assisted clinical tools (e.g., Gemini, Open Evidence)
• Remote work readiness with stable high-speed WiFi
