We pair people-centered navigation with technology to make sure patients understand their care, stay connected to their providers, and get to the resources they need.

Responsibilities

  • Serve as the primary point of contact for patients after their intake visit
  • Help patients understand and act on their care plan, including scheduling appointments and finding in-network providers
  • Resolve navigation friction regarding portals, referrals, appointment logistics, and provider communications
  • Identify practical and social-needs support such as food, housing, and transportation and connect patients to resources
  • Coordinate with clinical team members while providing non-clinical support only
  • Document every patient interaction and support activity in the platform accurately and on time
  • Maintain confidentiality and comply with HIPAA and privacy standards

Requirements

  • Active, unrestricted LPN/LVN license, Community Health Worker (CHW) certification, or equivalent patient-navigation experience
  • 2+ years of patient- or community-facing support experience
  • Working knowledge of the U.S. healthcare system and Medicare
  • Ability to work independently and manage a caseload
  • Reliable computer, internet, and a quiet space for patient calls

Preferred Qualifications

  • Experience supporting older adults or patients with complex or serious illness
  • Familiarity with Medicaid and other public programs
  • Bilingual skills in Spanish, Mandarin, or other languages common among Medicare patients
