Care Coordinator

Responsibilities

• Support CCM patient enrollment and onboarding activities
• Verify patient eligibility based on program guidelines
• Conduct weekly text and telephonic outreach to enrolled patients
• Reinforce provider-directed care plans during interactions
• Document patient interactions accurately within EHR and CCM platforms
• Track engagement time for Medicare billing compliance
• Facilitate patient access to referring providers and clinical staff
• Coordinate referrals and follow-up appointments
• Escalate symptom concerns or medication issues to licensed clinical staff

Requirements

• Certified Medical Assistant
• 1–3 years of healthcare experience
• Strong clinical assessment and documentation skills
• Proficiency with MS Office Suite (Outlook, Teams)
• Ability to work 8:00am – 5:00pm in the Central Time Zone
• Reliable home internet (min 25 Mbps download / 10 Mbps upload)
• Dedicated, interruption-free workspace for HIPAA compliance

Preferred Qualifications

• Experience in chronic disease management or nephrology
• Prior experience in virtual care, remote nursing, or telehealth
• Familiarity with CCM regulatory requirements
• Skills in patient education and motivational interviewing
