Job description

Responsibilities

  • Perform telephonic outreach to enroll and engage patients in clinical programs
  • Develop and implement care plans under the direction of a Nurse Care Manager
  • Facilitate seamless transitions of care across different healthcare settings and dialysis centers
  • Conduct patient assessments to identify social, economic, and environmental factors impacting health
  • Connect members to resources such as housing assistance, transportation, and food security
  • Advocate for members to identify and remove barriers to care
  • Maintain accurate and timely documentation of member interactions and care plans
  • Provide educational resources to empower patients and families

Requirements

  • 3 years of experience in healthcare coordination or community health work
  • Experience with a team-based approach to care
  • Excellent working knowledge of community resources
  • Ability to prioritize and task-switch effectively
  • Intermediate skills with MS Office Suite (Outlook, Teams)
  • Ability to work in a remote environment with a dedicated, private workspace
  • Minimum internet speed of 25 Mbps download and 10 Mbps upload
