Care Coordinator

Responsibilities

• Perform telephonic outreach to enroll and engage patients in clinical programs
• Develop and implement care plans under the direction of a Nurse Care Manager
• Facilitate seamless transitions of care across different healthcare settings
• Conduct patient assessments to identify social, economic, and environmental factors impacting health
• Connect members to resources such as housing, transportation, and food security
• Advocate for members to identify and remove barriers to care
• Maintain accurate and timely documentation of member interactions and care plans
• Provide educational resources to empower patients and families

Requirements

• 3 years of experience in healthcare coordination or community health work
• Experience with a team-based approach to care
• Excellent working knowledge of community resources
• Proficiency with MS Office Suite including Outlook and Teams
• Ability to work in a remote environment with required internet speeds (25 Mbps down/10 Mbps up)
• Ability to work 8:30a - 5p PST
