Care Coordinator, LPN

Responsibilities

• Perform monthly CCM touchpoints and build relationships with patients and caregivers
• Conduct structured symptom assessments and evaluate changes in condition using clinical judgment
• Identify clinical red flags and escalate urgent issues via defined triage pathways
• Collaborate with provider teams on symptom changes and medication-related issues
• Reinforce provider-developed CKD and chronic disease care plans
• Document all patient interactions accurately in EHR and CCM platforms
• Maintain high engagement and billable conversion metrics

Requirements

• Active LPN or Licensed Vocational Nurse (LVN) license
• 1–2 years of clinical experience in ambulatory care, population health, care coordination, or case management
• Strong clinical assessment, communication, and documentation skills
• Proficiency with EHR systems and MS Office Suite
• Ability to work in a remote environment with a dedicated, private workspace
• Reliable high-speed internet (min 25 Mbps download / 10 Mbps upload)

Preferred Qualifications

• Experience in chronic disease management
• Prior experience in virtual care, remote nursing, or telehealth
• Familiarity with CCM regulatory requirements
• Bilingual in Spanish and English
