Job description

Responsibilities

  • Consult directly with patients, caregivers, and care teams to implement pharmacy care management programs
  • Perform medication reconciliation and patient education
  • Utilize Comprehensive Medication Management (CMM) to optimize clinical aspects of medication therapy
  • Identify unnecessary drug therapies, renal dose adjustments, and contraindications
  • Engage in patient outreach to support medication adherence and behavior modification
  • Drive clinical quality initiatives including GDMT optimization
  • Participate in medication safety initiatives to minimize nephrotoxic medication use
  • Support appropriate medication use during post-hospital discharge periods
  • Analyze and recommend cost-savings strategies based on formularies and insurance
  • Act as an internal subject matter expert for patients and internal teams

Requirements

  • Doctorate or bachelor’s degree in pharmacy
  • Unrestricted Pharmacy state license (or ability to acquire within 60 days)
  • Strong comprehension of pharmaceutical therapies
  • Ability to deliver educational seminars to clinicians and patients
  • Intermediate skills with MS Office Suite (Outlook, Teams)
  • Ability to work in a remote environment with required internet speeds (25 Mbps down/10 Mbps up)
  • Dedicated, interruption-free workspace for HIPAA compliance

Preferred Qualifications

  • Residency training in ambulatory care, health system, or community pharmacy
  • BPS board certification in pharmacotherapy, ambulatory care, or geriatrics
  • Multistate licensure (TN, MN, CO)
  • Experience in care management, value-based care, or population health
