Responsibilities

• Conduct person-centered planning to capture client strengths and goals
• Manage budgets and advocate for support needs to local authorities
• Guide clients and partner organizations through the service process

Requirements

• Bachelor's Degree or equivalent work experience
• Social work or case management experience in disability or social services
• Experience managing multiple client-facing relationships

Preferred Qualifications

• Bilingual fluency in Spanish
• Experience with the California developmental disability system
• 2 years of case management experience
