Job description

Responsibilities

  • Ensure a supportive, positive experience for clients and referral sources
  • Work directly with clients, families, and referral sources to understand needs
  • Make accurate and timely outbound referrals for those not admitted
  • Collaborate with internal clinical and admissions teams
  • Document all client and referral source interactions in the electronic record system
  • Function as a liaison between Charlie Health and partners
  • Adhere to policies and achieve performance metrics goals

Requirements

  • Bachelor’s degree in health sciences, communications, psychology, social work, or related field
  • 1-2 years of relevant healthcare experience (e.g., case management, admissions, or outreach)
  • Strong interpersonal and relationship-building skills
  • Metrics-oriented mindset with experience meeting KPIs
  • Excellent written and verbal communication skills
  • Proficiency in Salesforce and Google Suite or MS Office
  • Work authorization in the United States
