Case Management Social Worker

Responsibilities

• Coordinate care across various settings including inpatient, outpatient, post-acute, ER, and home care
• Partner with members and providers to develop unique care plans and navigate the healthcare system
• Assess and support Social Determinants of Health (SDOH) needs and community resource requirements
• Participate in crisis intervention for members
• Serve as a member advocate to address barriers to care or compliance
• Communicate via phone and secure messaging to provide education on health conditions and medications

Requirements

• 2+ years of medical social work experience in a healthcare facility setting
• 1+ years of experience in care coordination and navigation
• Master of Social Work (MSW) from a nationally accredited school

Preferred Qualifications

• Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW)
• Behavioral health experience
• Case management experience
• Complex Case Management certification
• Bilingual skills (Spanish)
