Job description

Responsibilities

  • Coordinate care for mental health, behavioral, and social needs
  • Facilitate communication between patients, families, healthcare professionals, and community agencies
  • Provide psychological assessment, crisis intervention, and case management
  • Implement care plans and coordinate behavioral health and social services
  • Assist with insurance verification and authorization procedures
  • Develop and implement complex patient transition and discharge plans
  • Act as a resource for patients, referral sources, and division members

Requirements

  • Bachelor's Degree in Social Sciences
  • Skills in patient rights and teamwork
  • Ability to work in a clinical environment
