Certified Medical Assistant - Patient Care Coordinator

Responsibilities

• Serve as the primary point of contact for patients, families, facility partners, and providers
• Triage and prioritize urgent care requests and time-sensitive clinical communications
• Maintain accurate, compliant patient documentation within EMR systems
• Manage appointment scheduling, follow-up care, and interdisciplinary communication
• Lead new patient onboarding and ensure contact within 24 hours of enrollment
• Participate in after-hours on-call rotation
• Contribute to quality improvement and operational efficiency projects

Requirements

• 2+ years of direct patient care experience (MA, LVN/LPN, CNA, Phlebotomist, or similar)
• Experience in healthcare coordination, patient services, or clinical operations
• Proficiency with EMR/EHR systems and healthcare technology
• Strong triage and prioritization skills in high-volume environments
• Exceptional communication skills for diverse stakeholders
• Ability to work independently in a fully remote setting
• Reliable high-speed internet and a private, HIPAA-compliant home workspace
• Must maintain good standing with OIG, CMS, and Federal Health Care Programs
